Child’s medical condition

We require a copy of a hospital letter to verify the child’s medical condition. This must include

• Hospital address/details
• The child’s name, address and DOB
• Date of the appointment
• The child’s diagnosis

This can be uploaded in the form of a photo or a scan of the document.

N.B. In line with GDPR we do not keep or store these documents. Once a gift application has been considered the document will be deleted. Holly’s Hearts may seek confirmation from the hospital or agency if it is deemed necessary.

Gift selection

You can apply for up to 3 gifts in each application. Please put your preferred gifts in rank order and if applying for tickets or a break please state how many people this would be for e.g. 2 adults, 2 children. The most recent gift list can be found on our website and social media pages. If you would like something not on the list but of similar value please state this on your application being specific to what you would like and we can try to accommodate. We will contact you by email if your application is successful. Unfortunately there will be some applications that won’t be successful for a number of reasons. This could be because we have had a high number of applications, you haven’t submitted the correct documents, you do not meet our criteria or you have received a gift from us already in the last 12 months.
